Informed Consent, Assumption of Risk, Release of Liability & Client Agreement

The Client voluntarily chooses to participate in personal training, strength and conditioning, powerlifting instruction, fitness coaching, online coaching, nutrition education, fitness assessments, mobility training, group classes, or other services offered by Iron Geisha LLC.

The Client certifies that they:

• Are physically able to participate in exercise.
• Have consulted a physician if necessary.
• Will disclose any injuries, illnesses, surgeries, medications, pregnancy, or medical conditions that could affect participation.
• Agree to immediately notify the trainer of any changes in their health.

The Client understands Iron Geisha LLC does not provide medical advice or diagnose medical conditions.

The Client understands participation in fitness activities involves inherent risks including but not limited to:

• Muscle soreness
• Sprains
• Strains
• Falls
• Joint injuries
• Broken bones
• Equipment failure
• Heat illness
• Heart attack
• Stroke
• Paralysis
• Death

The Client voluntarily assumes all known and unknown risks associated with participation.

The Client agrees to:

• Follow all safety instructions.
• Use equipment properly.
• Inform the trainer immediately if pain, dizziness, shortness of breath, or discomfort occurs.
• Exercise within their physical limits.
• Stop exercising if instructed by the trainer.

To the fullest extent permitted by law, the Client releases and forever discharges Iron Geisha LLC, its owners, officers, employees, contractors, coaches, volunteers, affiliates, successors, and assigns from any and all liability, claims, demands, causes of action, damages, losses, or expenses arising out of participation in Iron Geisha LLC services, except where prohibited by applicable law.

The Client agrees to indemnify and hold harmless Iron Geisha LLC from any claims, lawsuits, damages, attorney's fees, or expenses resulting from the Client's participation or failure to follow instructions.

The Client understands that online coaching and app-based programming cannot monitor every exercise performed. The Client accepts full responsibility for:

• Exercise technique
• Equipment selection
• Workout environment
• Training decisions made outside direct supervision

The Client agrees to inspect equipment before use and immediately report damaged or unsafe equipment. The Client understands improper use of equipment may result in injury.

If an emergency occurs, the Client authorizes Iron Geisha LLC to contact emergency medical services. The Client accepts full financial responsibility for any medical treatment received.

Payment is due before services are provided unless otherwise agreed. Missed appointments, cancellations, refunds, and package expiration are subject to Iron Geisha LLC's current policies.

Please initial one:

• I authorize Iron Geisha LLC to photograph or record me for marketing, educational, or promotional purposes.
• I do NOT authorize Iron Geisha LLC to use my image.

Iron Geisha LLC will make reasonable efforts to protect personal information collected during coaching.

This Agreement shall be governed by and interpreted under the laws of the State of Texas.

If any provision of this Agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

This Agreement represents the complete understanding between Iron Geisha LLC and the Client and supersedes all prior verbal or written agreements.
